Shared Living Provider (Private Contractor)

NorthStar Services provides services to people with intellectual and developmental disabilities throughout Northeast Nebraska. NorthStar office locations are Bloomfield, Columbus, Fremont, Norfolk, O’Neill, Valentine, Wayne, and West Point. NorthStar provides services to these areas and surrounding areas.

A Shared Living Provider shares their home with a Participant and is responsible for the following services to the participant:

1. Providing and documenting habilitation, medication administration, medical care and appointments
2. Support and assistance with community involvement, healthy and/or therapeutic meals, personal care, transportation, finances, phone/communication, leisure and free-time activities, interpersonal relationships and family roles, reciprocity activities, and establishing or maintaining a daily routine
3. Support and assistance with a supportive, safe, and healthy living environment

NorthStar takes pride in matching quality Shared Living Providers with Participants to create a supportive environment for everyone involved.

NorthStar provides the following services to the Shared Living Provider and the Participant:

1. Annual Background check and associated costs, including One Source, APS/CPS registry checks and Medicaid Exclusion
2. Providing required training
3. Completing HCBS reviews
4. Assisting in acquiring an NPI# and troubleshooting in Maximus
5. Verifying provision sheets and entering online for payment
6. Providing Quality Assurance activities and oversight
7. Emergency Assistance
8. Access to NorthStar Nurse and Nurse oversight
9. Writing programs/Healthcare Staff Objectives/Mental Health Staff Objectives, conducting monthly reviews on all documentation and handling any necessary revisions
10. Monthly monitoring of the Shared Living setting
11. Providing back up staff (staff to fill in for the provider as needed) including all back-ground and training needed
12. Providing support staff (provides services alongside the provider at the same time)
